ABSTRACT

To measure the structure relationship among indexes and different indexes groups based on the networkrelationship information. Methods: Tabu search algorithm was used as the concrete algorithm to study the composition ofindex groups and the structure relationship between them. Result: The results of simulation and real data show that Tabusearch algorithm accurately solve the division of indexes and measure the structure relationship among different groups.Conclusion: Tabu search algorithm measure the structure relationship among indexes and different indexes groups, whichextend to larger network and more complex situations.

ABSTRACT

This paper was aimed to understand the clinical features of traditional Chinese medicine (TCM) through the analysis of relevant information among fracture patients treated by the Cervus and Cucumis Polypeptide Injection (CCPI).The study was conducted among fracture patients treated by CCPI in the hospital information systems (HIS) among 26 national triple-A hospitals from 2003 to 2014.The general information,TCM syndrome and solar term of morbidity were analyzed.The results showed that the number of patients from 45-64 years old were the most,accounting for 39.84%.Among them,the number of male patients from 18-44 years old was the most,accounting for 43.38%;the number of female patients from 45-64 year old was the most,accounting for 44.33%.The main TCM syndrome of admission and discharge was qi stagnation and blood stasis,which accounted for 60.53% and 53.08%,respectively.Beginning of Summer (7th solar term) and Rain Water (2nd solar term) were solar terms with the most patients,which accounted for 5.26% and 5.24%,respectively.It was concluded that the majority of fractures patients treated with CCPI were the elderly.The syndrome of qi stagnation and blood stasis was the most.The hospital admission of most patients was at the Beginning of Summer and Rain Water.It provided theoretical basis to pay attention on TCM syndrome differentiation in the treatment of clinical fractures.The treatment should be adapted to the characteristics of the disease itself,the change of solar terms,and dialectical medication.

ABSTRACT

This article mainly introduces the functional clustering methods and demonstrates its performance by the real analysis of Chinese medical Zong Qi data.The functional clustering analysis hypothesizes that the discrete time series observations are dominated by a continuous function of time,which can be expressed by infinite basis functions.Functional clustering methods include raw data method,filtering method and adaptive method.When dealing with the sparse data clustering analysis,raw data method encounters the difficulty of matrix calculation due to the lack of data on some time grids.Filtering method suits for full time data,while when facing missing data,the fitting curve is inaccurate so that the clustering outcome cannot be explainable.Adaptive method can be applied flexibly to both full time and sparsely sampled data.In the real analysis section,the adaptive method is used to cluster the sparsely sampled Chinese medical Zong Qi time series data,where the elderly individuals are divided into three clusters,the ones with high level of Zong Qi,the ones with moderate level and those with low level.The adaptive method performs well on clustering individuals.

ABSTRACT

Objective In observational studies or non-randomized design,the researchers' ability to make causal inferences from data was hampered by confounding factors.This study used this method to analyze a group of observational medical data in order to instruct relevant medical personnel to carry out their own causal inference studies.Methods At present,the four main types of propensity scoring methods:matching,stratification,inverse probability weighting and covariate adjustment have been widely used in the study of causal inference.Propensity score method can theoretically eliminate the bias of the observable confounding factors,so that the treatments variables are close to the result of random assignment design,thus,it is estimated that the treatment factor has a causal effect on the outcome.Results Considering the advantages of the inverse probability weighting method over other methods,this paper summarizes the applicable conditions for the estimate of causal effect,particularly illustrates the use of a modern nonparametric statistical technology--Generalized Boosted Models (GBM) and its advantages and disadvantages.Conclusion When there is a lot of different types of confounding factors,and uncertain functional forms for their associations with treatment selection in linear,non-linear or interaction effect,and other issues,GBM propensity score weighting method can overcome the obstacles in the process of accurately estimating propensity score.

ABSTRACT

Cox regression model is one of the most widely used methods in the survival analysis.One assumption of this model is that there is no tie in the failure times,that is,individual has different failure times.In practical applications,the existence of ties in time data is very common.In this paper,four common methods of dealing with ties in Cox model,including Exact method,discrete model method,Efron method and Breslow method,were compared with simulation.The results showed that Exact method and discrete model were the best,but they took the longest time.Efron method and Breslow method were faster but there was a greater deviation in parameter estimation.Moreover,the sample amount and ties degree also affect the results.In general,when there are a few ties,the difference between four methods was small;and in the case of large datasets or a large number of ties,the bias of three approximation methods increased except Exact method.However,there was no significant change on computational time.While the computational time of the Exact method increased rapidly.Therefore,if the estimation precision is not as important as the estimation time,Efron method and Breslow method will be good choices.Efron method is more preferably as it is more precise.And Breslow method tends to underestimate the true β.If there is no limit in time,Exact method and discrete model can be chosen to achieve more accurate results.

ABSTRACT

Multiple outcomes measured repeatedly for the same subject are common in longitudinal observation.If we use the approach by analyzing each outcome separately,it may lead to wrong conclusions due to the failure of accounting for joint evolution of different outcomes.To adequately capture the interdependence among multiple outcomes,we proposed a joint modeling for multivariate longitudinal data by constructing a linear mixed-effects model for each outcome and accommodating the relationship among multiple outcomes through correlation in random effects.Maximum likelihood method was adopted to estimate parameters in this model.The application of this method was demonstrated through the analysis of stroke data.

ABSTRACT

Objective To explore characteristics of the elements of syndrome, the disease position and the relationship between chemical indicators and TCM syndromes of type 2 diabetic encephalopathy. Methods 2 501 cases of type 2 diabetes clinical data were collected from Guang'anmen Hospital of China Academy of Chinese Medical Sciences, Beijing University of Chinese Medicine, Dongzhimen Hospital, Dongfang Hospital, etc. in nearly 3 years, among which, 342 cases were type 2 diabetic encephalopathy. The original clinical data were double entried in epidata by two people, establishment forms in excel, factor analysis and Bayesian networks were used as data mining research methods. Results 20 elements which characteristic root more than 1 were derived by factor analysis, 68.4% were covered. Of all 20 elements, five factors belong to Yin, five factors belong to blood stasis; lassitude, shortness of breath, stool frequency were appeared when fasting glucose abnormalities; lassitude, hemiplegia were appeared when 2-hour postprandial blood glucose abnormalities;lassitude, feverish palms and soles, stool frequency, more nocturnal enuresis when glycated hemoglobin abnormalities by Bayesian networks. Conclusion The Elements of the syndrome of type 2 diabetes encephalopathy were deficiency of Yin and blood stasis; and the main positions for diabetic patients were liver, spleen and kidney. Patients with impaired fasting glucose were Qi deficiency; Patients with impaired 2-hour postprandial glucose were Qi deficiency or pathogenic wind attacking collaterals; Patients with abnormal hemoglobin were Qi deficiency and Yin deficiency.

ABSTRACT

<p><b>OBJECTIVE</b>To verify the efficacy and safety of post-marketed fleabane injection combined with Dengzhan Shengmai capsules in the treatment of ischemic stroke (IS).</p><p><b>METHOD</b>A multicentre, prospective, practical, randomized controlled study was carried out to compare the efficacy and safety of Dengzhan group (n = 343) and western medicine group (n = 335), appling "clinical study central stochastic system". The treatment of Dengzhan group is using fleabane injection in acute stage and Dengzhan Shengmai capsules in convalescence. The primary indexes of effect evaluation are the important outcome events in 360 days' follow-up, including mortality, recurrence, disability and quality of life to reflect the effect of clinical study. The indexes of safety evaluation involve laboratory examination results and incidence of adverse events.</p><p><b>RESULT</b>After 360 days' follow-up, 4 people died of IS in Dengzhan group, and the mortality rate of which is 1.17%, while 16 died in Western medicine group (WM group), and the mortality rate is 4.78%, suggesting that the mortality rate of Dengzhan group is significantly lower than WM group (P<0.05). Eleven cases recurred in Dengzhan group, and the recurrence rate of which is 3.21%, while 12 recurred in WM group, and the recurrence rate is 3.59%, indicating that the recurrence rate of Dengzhan group is slightly lower than WM group. The disability rate of Dengzhan group is 39.53%, among which the rate of severely disabled cases are 1.49%, while the disability rate of WM group is 40.13%, among which the rate of severely disabled cases are 3.13%, suggesting that the disability rate of Dengzhan group is lower and the severity of disability is also lighter than WM group. In the field of quality of life, the activity ability and the upper limb function store of stroke patients in Dengzhan group improved far much better than WM group (P<0.05). Analysis of safety suggested that, adverse events occurred in 11 cases in Dengzhan group, among which 4 cases is related with the drug treatment, the incidence of adverse events of which is 1.17%, and the main manifestations involve fever and chilling, rash, nausea, dizziness, palpitation, etc. which were all appeared after the treatment of fleabane injection, and disappeared 1 to 2 days after drug withdrawal. 13 cases occurred abnormal liver function and 2 cases abnormal kidney function in Dengzhan group. According to the judgment of clinical physicians, 3 case of ALT abnormality is possibly related to the treatment, the others are all unrelated with the treatment.</p><p><b>CONCLUSION</b>Fleabane injection and Dengzhan Shengmai capsules are all safe and effective TCM in the treatment of ischemic stroke.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect and safety of Kudiezi injection on patients with acute ischemic stroke.</p><p><b>METHOD</b>Seven hundreds patients were divided into two groups by central randomization system. The study group, 346 cases, was treated with kudiezi injection plus traditional Chinese medicine (TCM) synthesis rehabilitation project, and the control group, 354 cases, was treated with synthetic rehabilitation project. The patients were treated for 10 to 21 days. Before treatment and at the 7th, 14th and 21th day of treatment, the indexes include NIHSS used for evaluating the neurological deficit degree and the motor function score (Fugl-Meyer) for evaluating motor function were observed. The safety index is defined by adverse observation event and laboratory test. The incidence of adverse events and laboratory tests results were observed before and after treatment at the same time.</p><p><b>RESULT</b>Application of generalized estimating equation model, we found that as the treatment time, NIHSS score and FMI score of the two groups showed a trend of improvement. And at the 14th days and 21th days of treatment, compared to the control group the treatment group showed significant statistical difference on the impact of NIHSS and FMI (P<0.05). No serious adverse events were observed.</p><p><b>CONCLUSION</b>Kudiezi injection plus TCM rehabilitation project of ischemic stroke showed some superiority to western medicine rehabilitation program on improving the neurological deficit and motor function. Kudiezi injection is safe and effective in the treatment of acute ischemic stroke.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the clinical application of Shenmai injection and to learn the rule of it in the real word so that we can provide inferences to doctors.</p><p><b>METHOD</b>Cases using Shenmai injection in hospital information system from 10 grade A hospitals in Beijing were collected. Descriptive statistic methods were used to analyze the usage, dosage and medication time.</p><p><b>RESULT</b>For the clinical application of Shenmai injection, the average age of the patients using Shenmai injection was about 64.68, but most of them came from cardiovascular diseases department Shenmai injection was usually used to treat coronary disease in Western medicine and Xiongbi (Chinese medicine syndrome name) in traditional Chinese medicine. The course of treatment was from one day to seven days, which was in accord with the drug instruction as well as the dosage.</p><p><b>CONCLUSION</b>Shenmai injection was usually used for the old people. In clinical practice, the use of Shenmai injection was basically in accord with the drug instruction by doctors. However, there are still some of the doctors who did not follow the instruction and should be careful of this.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To know how Danhong injection is used in clinical practice and to provide a reference for guiding clinical use of Danhong Injection.</p><p><b>METHOD</b>Extract Danhong injection's post-marketing re-evaluation data from the Hospital Information System of ten three grade III-A General Hospitals in Beijing, use basic statistical analysis methods to analyze Danhong injection's indications, usage and dosage, days of treatment etc. in clinical practice.</p><p><b>RESULT</b>In patients using Danhong injection, there were more than 60 percent patients were prescribed based on main-diagnosis, first-visit and other diagnosis, which were also coincided with Danhong injection's instruction. In clinical practice, 95.5 percent of Danhong injection's administration routes conformed to the instruction and more than 90 percent of Danhong injection's dosage were within the limits prescribed by the instruction. Danhong injection was used less than 20 days successively. Danhong injection was used in combination with drugs with the action of removing blood stasis, antianginal drug, antiplatelet drug, drugs for cerebrovascular disease and so on.</p><p><b>CONCLUSION</b>Danhong injection has been used according to instruction in practice almostly.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the rules of medicinal groups that doctors treat angina pectoris using Danhong injection.</p><p><b>METHOD</b>Two thousands and seventy cases from hospital information system of 10 hospitals in Beijing 3a grade hospital were collected. Then use frequencies to find the most common drug and use association rules with apriori algorithm to analyze doctor's prescription compatibility rules curing coronary heart disease.</p><p><b>RESULT</b>The compatibility among aspirin, oral nitrate, oral dose of clopidogrel and oral dose of Xintong was the most frequently used medicinal groups with Danhong injection in real world HIS database.</p><p><b>CONCLUSION</b>Danhong injection was used at treatment of coronary heart disease is often combination with similar pharmacological effects chemical drugs. Doctors should balance the benefits and the risks.</p>

ABSTRACT

With the continuous improvement of international "pharmacovigilance" technology and methods,it becomes the key part of the post-marketing evaluation. This issue is based on this research background, and also means to find out the Chinese medicine safety monitor which consistents with the reality. A common problem is that those who choose a career in pharmacovigilance know how the complex data presented to us are a source of both fascination and frustration. In the 70's, for the first time data mining technology in the international pharmacovigilance turn up, we try to establish new signal detection method to make contributes to post-marketing evaluation of Chinese medicine and establishment of registration. Building the national adverse reaction reporting database is widely used in western country. Nature of the problem is that pharmacovigilance issues can come through a lot of assumptions into the statistical problems, different assumptions are for different statistical tests. Through the traditional imbalance between the proportion of fourfold table for other assumptions, few countries use in practice, this does not involve evidence, but this issue provides the introduce of the principle. Methods include the ratio of the report of the Netherlands (ROR), the proportion of reports than the UK ratio (PRR),WHO's information points (IC), the U.S. Food and Drug Administration empirical Bayes (EBS), etc. Because there is no international gold standard of the signal detection method, at first we use the simulation comparing these four methods of data mining, From the point of specificity, the sample size demand, this issue views the advantages and disadvantages of four methods and application conditions,and from a technical point of view and try to propose a new signal detection method, for example, Hierarchical Bayesian.

ABSTRACT

With the continuous increasement of adverse Chinese drug reaction reporting, especially Chinese traditional medicine injection, it becomes the key part of the post-marketing evaluation. The issue based on this research background becomes the hot research direction. But the truth is that the history of Chinese medicine and western medicine is different and the theory is not a system either, here different means between pharmacovigilance of Chinese medicine and western medicine, there are still many gaps. This becomes a barrier for Chinese medicine entering the international market. This article summarizes try to order the SFDA's adverse reaction reporting database of traditional Chinese medicine, and using three kinds of data mining methods used by WHO, the U. S. Food and Drug Administration, to establish suitable system for Chinese market after the evaluation of pharmacovigilance.

ABSTRACT

Objective To epidemiologically analyze the common clinical symptoms of sub-health in Beijing area.Methods By analysis of sub-health,epidemiologlcal data and the method of factors analysis, we studied the features and properties of various symptoms and their relationships that represent various kinds of sub-health.We used the structural equation modeling to validate the relationship between the symptom groups and between the symptom groups and their components.Results The amount of the factors whose weight Values were larger than 1.0 was 13,and the cumulative value of all the factors was 55.67%.With the domain knowledge,however,the total factors were merged into 10.We took fatigue as the main hidden variable to build a structural equation model.The function was x2/df=3.65,and GFI,AGFI,CFI,Tucher Lewis indexes were all above 0.9.The direct effects between the hidden variables showed that fatigue is closely related to fatigue of eyes,sleep,and pain with the weighted coefficient ranging from 0.7 to 1.0. Fatigue is somehow related to the symptoms of stool,pee,fauces,mouth,and sweat with the weighted coefficients about 0.5.The indirect effects between fatigue and hidden vailables showed that fatigue has an effect on moods indirectly with standardized indirect effects and standardized total effects was 0.702 and 0.112.respectively.Conclusions The symptoms of sub-health are complex with a dominating symptom and many other secondary symptoms.The common symptom groups are maladjustment of mood,fatigue, abnormity of sleep,maladjustment of stool and pee,double-minded,fatigue of eyes,and pain.The results of structural equation model shows that the causality between symptoms that are in a symptom group is different,so is the causality between the symptom groups,which shows that the symptoms of the sub-health are of diversity.

ABSTRACT

Objective To analyze the central effect of Qi-Yin Deficiency Syndrome (QYDS) scoring in clinical trials of diabetes.Methods Dispensation degree analysis and multiple linear regression analysis were adopted to compare the differences of central effects among the Qi-Yin deficiency syndrome scoring,fasting plasma glucose (FPG) level and 2-hour postprandial blood sugar (2 hPG)level before and after treatment in the suited databases from two phase Ⅲclinical trials of type two diabetes performed in 2004~2005.Results The variation coefficients of QYDS scoring treated with drug A and drug B varied from one tenth to half times of those of FPG and 2 hPG levels before and after treatment.And the influence of centers on QYDS is weaker than those on blood sugar levels (FPG &2 hPG) either referring to the numbers of significant centers or referring to the absolute values of standard regression coefficients in multiple linear regression equation.Conclusion There exists a central effect in QYDS scoring before and after treatment,and the central effect of QYDS scoring is weaker than that of blood sugar levels (FPG &2 hPG) in clinical trials of diabetes.

ABSTRACT

Objective To explore the relationship among the disease - syndrome - formula of type 2 diabetes.Methods Based on the clinical data of 2501 cases of type 2 diabetes, to study the relationship among the disease - syndrome - formula of type 2 diabetes by making use of association rules.Results Three formulas, Shengmai Yin (Pulse-activating Fluid), Liuwei Dihuang Decoction (Decoction of Six Herbs Including Rehmannia), and Sijunzi Decoction (Decoction of Four Noble Herbs), were selected from 122 ones to analyze the relationship between the formula and symptoms. The occurrence of fatigue with weakness, dry mouth with thirst, frequent urination with increased volume of urine, numbness of limbs, blurred vision, thirst with preference of drinking water, frequent nocturnal urination, and shortness of breath with inactive talking was in high frequency in those patients who were prescribed Shengmai Yin. The occurrence of fatigue with weakness, thirst with preference of drinking water, blurred vision, frequent nocturnal urination, dry mouth with thirst, numbness of limbs, shortness of breath with inactive talking, and soreness and weakness of lumbus and legs was in high frequency in those patients prescribed Liuwei Dihuang Decoction, and there existed a multi-association among these symptoms. The occurrence of fatigue with weakness, soreness and weakness of lumbus and legs, frequent nocturnal urination, shortness of breath with inactive talking, numbness of limbs, insomnia, and blurred vision was in high frequency in those patients who were prescribed Sijunzi Decoction, and there existed a multi-association among these symptoms.Conclusion The association rule can be adopted to analyze the relationship between formula and syndrome, and to explore the multi-association relationship among the symptoms of type 2 diabetes.
